Two patients came to my OPD on the same day. Both said, “Doctor, there is a blockage in the veins of my leg.” But the first patient's leg was cold as ice and pale, and the second patient's leg was swollen, red and hot.
Same complaint, opposite problems. The first had a blocked artery, and that is what this page is about: why it happens, especially in diabetes, the warning signs, and how angioplasty can save the leg.

Two pipes in your leg: supply line and drainage line
Your leg has two kinds of pipes. Arteries bring fresh, oxygen-rich blood from the heart to the leg. They are the supply line. Veins carry used blood back to the heart. They are the drainage line.
Think of your house. If the tap stops, the plants dry up. If the drain blocks, the house floods. It is exactly the same in your leg.
Why do arteries get blocked?
Over the years, fat and cholesterol build up on the artery wall. This is called plaque. Four things make plaque grow fast: diabetes, smoking, high blood pressure and high cholesterol.
In diabetes, the small arteries below the knee are often affected, and reduced sensation in the feet means a small injury may go unnoticed. That combination of poor blood flow and an unnoticed wound is why the diabetic foot is so dangerous.
Warning signs: do not ignore these
- Pain in the calf or thigh after walking a certain distance, which goes away when you stop. This is often the first sign.
- Cold, pale foot, or a weak or absent pulse in the foot
- Pain in the foot at night or at rest, better when the leg hangs down
- A wound on the foot that does not heal in a few weeks
- Toes turning blue or black
Diagnosis: simple and painless
A Doppler ultrasound uses no needles and causes no pain, and it shows where the artery is narrowed or blocked. If treatment is needed, a CT angiography or an angiogram maps the arteries in detail.
Treatment: angioplasty to save the leg
For many arterial blockages, major bypass surgery is not needed. Through a tiny hole, usually in the groin, a thin catheter is guided to the blockage under X-ray. A balloon opens the narrowed artery, and where needed a stent keeps it open.
When blood flow returns, the foot warms up, rest pain eases, and wounds get a real chance to heal. In the diabetic foot, angioplasty works best as part of a team with your diabetologist, wound care and good sugar control.
Angioplasty vs bypass surgery
| Angioplasty / stent | Bypass surgery | |
|---|---|---|
| Cut | No, only a pinhole | Yes, long cuts on the leg |
| Anaesthesia | Local | Usually general or spinal |
| Hospital stay | Usually 1–2 days | Usually a week or more |
| Recovery | Walking within a day | Several weeks |
| Can be repeated if needed | Yes | More difficult |
Some long or complex blockages still need bypass surgery. The best option depends on your scans and your overall health.
Prevention: protect your feet
- Check your feet every day for cuts, cracks, blisters or colour change, using a mirror if needed
- Keep sugar, blood pressure and cholesterol under control, and stop smoking completely
- Wear soft, well-fitting footwear, and never walk barefoot
- Walk regularly. It helps build blood flow in the leg
- See a doctor early for any wound on the foot
Mistakes to avoid at home
- Using a hot water bottle or heat pad on cold feet. In diabetes, sensation is reduced, the skin can burn without you feeling it, and the wound gets worse.
- Massaging a leg that is suddenly swollen and red. It could be a clot (DVT), and massage can move it.
- Treating a non-healing foot wound only with dressings for months, without checking the blood flow.
- Waiting for toes to turn black before seeing a specialist.

Questions patients ask me
Can a blocked leg artery be opened without surgery?
In many patients, yes. Angioplasty opens the artery with a balloon, and sometimes a stent, through a pinhole. Some long or complex blockages still need bypass surgery.
My diabetic foot wound is not healing. Could it be a blood flow problem?
Yes, poor blood flow is a common reason a diabetic foot wound does not heal. A Doppler test shows whether the arteries are blocked. Restoring blood flow can help the wound heal.
Can angioplasty prevent amputation?
Restoring blood flow early gives the foot its best chance to heal and can help avoid or limit amputation. Results depend on how early treatment starts and how much damage has already happened.
What is the first warning sign of leg artery blockage?
Pain in the calf after walking a certain distance, which settles when you stop and comes back when you walk again.
How long is the hospital stay after angioplasty?
Usually one to two days. Most patients walk within a day.
Is angioplasty for leg arteries covered by insurance?
Yes, it is covered by health insurance.
Pair thanda kyun padta hai aur chalne par pindli mein dard kyun hota hai?
Yeh pair ki artery mein blockage ka sanket ho sakta hai. Diabetes, smoking, BP aur cholesterol isse badhaate hain.
Blood flow theek ho toh ghaav bharne ka mauka badhta hai.
In short
- Arteries are the supply line. When they block, the foot turns cold and pale, walking hurts, and wounds don't heal.
- Diabetes, smoking, BP and cholesterol are the four big causes. Check your feet daily and never use heat on cold feet.
- Angioplasty through a pinhole can reopen the artery and give the leg its best chance. Don't delay.
